Terms and Conditions of Registration
3rd Heidelberg Quality Improvement Forum, Germany, 15/16 June 2017 („Forum“)
All registrations are subject to the terms and conditions set out below. evaplan GmbH am Universitätsklinikum Heidelberg’s (“evaplan”) registered address is Ringstraße 19b, 69115 Heidelberg, Germany.
- The Forum
1.1 The registration fee of the Forum includes:
– Entrance to the conference and workshops
– Snack lunch and networking reception with hors d’oeuvre on 15 June 2017
– Lunch on 16 June 16 2017
– Refreshments (non-alcoholic beverages and coffee/tea) on both days
– Conference materials
- Placing an order and our acceptance
2.1 When you register for the Forum, the languages that evaplan offers to you for the conclusion of this process will be English and German only.
2.2 For all registrations (including online registrations on www.qi-forum.org), submission of the registration is an offer which will be acknowledged in writing by email by evaplan. Registrations are only completed upon receipt of the registration fees on evaplan’s bank account (IBAN: DE64 6725 0020 0009 0285 87 BIC: SOLADES1HDB). If you wish to receive an invoice, you must contact evaplan.
- Travel & hotel arrangements
3.1 The Forum participants are responsible for making their own travel and hotel arrangements. evaplan merely suggests convenient hotels in the area of the venue. evaplan has no contractual relations with any of these establishments.
- Use of personal data
4.1 The information you supply on any registration form, will be used to process your request to attend the Forum, inform the venue and caterers of all delegates, create a delegate list of names for all attendees and generally manage your registration and fulfil our obligations to you. No information will be passed on to third parties or be used commercially.
- Prices, credit terms and payments
5.1 Prices are only valid for the 3rd Quality Improvement Forum and charged in euros (EUR), including 19% VAT.
5.2 Payment of the registration fees and invoice must be received on evaplan’s bank account specified on the Forum’s programme and invoice by the June 15, 2017 and should be by bank transfer. Payment by any other method should be agreed in advance.
- Cancellation, alterations, losses, refunds and liability
6.1 Transfers from one delegate to another for the booked conference can be permitted upon written notice to evaplan.
6.2 Registration fees cannot be refunded.
6.3 It may be necessary for reasons beyond the control of evaplan to alter the content and timing of the conference, the identity of the speaker, and/or the date or the venue. In the unlikely event of the conference being cancelled (as opposed to postponed) by evaplan, evaplan will refund you the registration fee to the fullest extent permitted by law, however, evaplan will not be liable to you for any other costs or losses, whether direct or indirect. This does not affect your statutory rights.
6.4 Notwithstanding the other provisions in this contract our liability shall be limited to the booking fee and evaplan shall not be liable for any indirect or consequential loss. Nothing within this contract shall be taken to exclude or limit our liability for death, personal injury, fraudulent misrepresentation or where such limitation of liability is otherwise not permitted by law.
- Venue rules
7.1 You will need to comply at all times with the rules and regulations imposed by the venue and may be excluded or removed from the conference for failure to adhere to such requirements. You will also need to ensure that you have your own insurances against any loss of your property or any loss or damage you may cause at the conference.
8.1 If you breach these conditions and evaplan take no immediate action, this will not be deemed a waiver of our rights and evaplan will still be entitled to take further action at a later time.
- Events beyond our reasonable control
9.1 Should evaplan be unable to perform any obligations under this Agreement due to causes or circumstances beyond its reasonable control, including but not limited to any force majeure such as, terrorism, fire, flood, drought, strike, lock out or actions of the venue, they will not be liable to you for any losses or claims arising from this.
- Entire Agreement
10.1 To the fullest extent permitted by law, these conditions constitute the entire agreement between you and evaplan with respect to the Forum being booked.
11.1 These conditions cannot be changed or terminated orally, and no changes or amendments to these conditions shall be binding until accepted by both you and evaplan in writing (which includes by fax and email).

- Governing Law and Jurisdiction
13.1 The terms and conditions are subject to German legislation. For all claims arising from the terms and conditions and all legal dispute, the place of jurisdiction is Heidelberg, Germany.
